The Global Cancer Observatory (GLOBOCAN) and the global burden of disease (GBD) are two key databases for cancer epidemiology. In this study, we aim to assess the consistency of genitourinary cancer incidence and mortality estimates from the same year across the two databases, analyze the association between observed discrepancies and levels of social development and available health resources, and explore the reasons underlying these differences. Age-standardized incidence and mortality rates (ASIR and ASMR) per 100,000 for kidney, bladder, prostate, and testicular cancers were extracted from GLOBOCAN and GBD for 2022. For each cancer, relative differences (RDs) and ranking differences in age-standardized rates (ASRs) were analyzed between the two databases. Spearman's coefficients were used to identify correlations between RD and levels of social development, as well as available health resources. Differences in data sources and methods were also examined. Discrepancies in ASRs were observed globally and across nearly all 178 countries studied. ASR discrepancies were negatively correlated with levels of social development and available health resources in most analyses. These differences stem from variations between the two databases in cancer definitions (e.g., kidney cancer), standard populations (Segi's vs. GBD world standard), and data sources and methods. Although the ASRs are derived from the same year, notable discrepancies exist between the two databases. The reliability and accuracy of GBD and GLOBOCAN data should be carefully assessed when interpreting disease epidemiology studies.
