ABSTRACT This prospective single‐arm observational clinical study evaluated intraoperative pain during root canal obturation using the Tagger hybrid thermomechanical compaction technique (THTCT) in necrotic teeth treated in a single visit. A total of 151 necrotic teeth from 151 patients aged 12–90 years (mean ± SD: 39.7 ± 17.9 years) were included. All teeth underwent standardised chemomechanical preparation and were obturated using the THTCT with EndoFill sealer. Immediately after thermomechanical compaction, patients were asked to rate their intraoperative pain intensity using a normalised pain scale. Demographic and clinical variables, including biological sex, tooth type, periapical diagnosis, and sealer extrusion, were also analysed. Intraoperative pain intensity was generally low and was not significantly associated with biological sex ( p = 0.251), tooth type ( p = 0.211), periapical diagnosis ( p = 0.733), or sealer extrusion ( p = 0.934). The THTCT was associated with minimal intraoperative pain, supporting its clinical safety, patient comfort, and cost‐effectiveness.
