Developmental delay can impact different domains of occupational performance, requiring interventions that consider the child in their entirety. Occupational Therapy stands out for using playful resources as mediators of development, promoting participation and engagement in activities. Children's books, as a playful resource, enable the integration of cognitive, motor, and socioemotional aspects. The International Classification of Functioning, Disability and Health (ICF) can be used to structure assessment, clinical reasoning, intervention strategies and outcome measurement, contributing to a better understanding of functioning. This study aimed to describe, through the ICF, the results of an occupational therapy intervention with a child presenting developmental delay, in which a children's book was used as a playful resource. This was an exploratory, qualitative case study with an intervention approach. The methods employed were participant observation, a structured field diary, two semi-structured interviews, and the ICF. At the end, improvements were observed in cognitive, emotional, affective, and language skills, as well as in body perception, resulting in satisfactory occupational performance and greater autonomy and independence. The study suggests the use of children's books as a playful resource in occupational therapy practice, although further investigations are needed.
