Relacorilant in Combination With Nab-Paclitaxel for the Treatment of Adults With Platinum-Resistant Epithelial Ovarian, Fallopian Tube, or Primary Peritoneal Cancer

Objective: To review the pharmacology, efficacy, and safety of relacorilant (Lifyorli) in combination with nab-paclitaxel (NP) for the treatment of adults with platinum-resistant epithelial ovarian, fallopian tube, or primary peritoneal cancer who have received 1 to 3 prior lines of systemic therapy, including at least one bevacizumab-containing regimen. Data Sources: A literature search was conducted using Embase and PubMed/MEDLINE, applicable published abstracts, and ongoing studies from ClinicalTrials.gov between January 1, 1981, and July 1, 2026. Keywords included relacorilant, Lifyorli, and CORT125134. Study Selection and Data Extraction: All English-language studies involving relacorilant for the treatment of cancer or other conditions were included. Data Synthesis: Relacorilant is a highly selective, nonsteroidal, reversible glucocorticoid receptor antagonist approved in combination with NP based primarily on a phase 3 clinical trial of 381 patients, in which relacorilant plus NP significantly improved progression-free and overall survival compared with NP alone. Serious adverse events (AEs) occurred in 35% of patients receiving the combination, although treatment discontinuation due to AEs was uncommon. Common AEs associated with the combination included neutropenia, anemia, thrombocytopenia, nausea, and fatigue. Relevance to Patient Care and Clinical Practice in Comparison With Existing Drugs: Platinum-resistant ovarian cancer remains a difficult-to-treat disease with limited therapeutic options. Unlike biomarker-restricted therapies, relacorilant plus NP improved survival outcomes in a heavily pretreated, biomarker-independent population. Conclusion: Relacorilant represents a novel strategy for reversing chemoresistance in combination with NP alongside a manageable safety profile for heavily pretreated patients with platinum-resistant epithelial ovarian, fallopian tube, and primary peritoneal cancers.
