From Screen to Care: Physician Approaches to Overcoming Communication Challenges in Telemedicine

Background: The growth of telemedicine has encouraged physicians to adapt their communication techniques for remote consultations. Even though telemedicine enhances access and continuity of care, it also presents barriers to effective doctor–patient interaction. Aims: This study explores the communication challenges faced by physicians in telemedicine consultations and identifies the strategies they use to ensure effective and high-quality virtual care. Methods: A qualitative descriptive study was conducted through semi-structured interviews with 12 practicing physicians. Interviews focused on their experiences conducting telemedicine consultations. Data were thematically analysed to identify recurring communication barriers and adaptive strategies. Results: Seven major thematic domains were identified: (1) visual professionalism, (2) technological readiness, (3) establishing trust, (4) building rapport, (5) information gathering, (6) clinical assessment adaptation, (7) shared decision-making and follow-up commitment. Physicians employed adaptive strategies including environmental adjustments, verbalized empathy, action narration, patient-guided self-examinations, and explicit planning of follow-up care. Conclusion: Telemedicine consultations require additional preparation and adjustments across various aspects of clinical practice to ensure effective implementation and maintain the quality of care.
