ENDODONTIC TREATMENT OF TEETH AFFECTED BY IATROGENIC DENTAL TRAUMA: 3-YEAR FOLLOW-UP

Introduction: Orofacial procedures can cause dental trauma, including intubation-related injuries that may lead to pulp necrosis and resorption. Treatment options include monitoring, extraction, or bioceramic repair, but the prognosis remains uncertain. Objective: To describe the non-surgical endodontic approach in maxillary central incisors affected by iatrogenic trauma, as well as the surgical intervention performed on a left maxillary central incisor with invasive cervical resorption. Case report: A 20-year-old patient, with no history of comorbidities, sought dental care due to intense pain in the anterior teeth one month after a tonsillectomy. Clinical analysis and radiographic examination revealed that teeth 11 and 21 had no restorations or visible cracks, exhibited a healthy periodontal condition, and showed no mobility or periodontal pockets. However, the presence of pain upon thermal stimulation and radiographic findings led to a diagnosis of symptomatic irreversible pulpitis. Endodontic treatment was performed, followed by definitive restoration. After six months, the patient remained asymptomatic. However, after one year, cone-beam computed tomography (CBCT) revealed the presence of invasive external cervical resorption in tooth 21, despite the absence of symptoms. Given this finding, a surgical endodontic approach was planned, including lesion curettage and filling with bioceramic cement. After one year of surgical follow-up, complementary examinations confirmed bone neoformation and stabilization of the invasive external cervical resorption process. Conclusion: The treatment was successful, with endodontic therapy and bioceramic surgery leading to bone neoformation and resorption stabilization. Continuous follow-up is crucial for long-term dental health preservation.
