Midwifery-coordinated care: Improving quality of life, maternal satisfaction, and medication adherence in pregnancy-induced hypertension

Objective: The objective of this study is to evaluate the impact of a midwifery-coordinated care model on quality of life, maternal satisfaction, and medication adherence among women with pregnancy-induced hypertension. Materials and Methods: A quasi-experimental, post-test–only control group design was employed. A total of 100 antenatal women diagnosed with pregnancy-induced hypertension (PIH) after 20 weeks of gestation were recruited using convenience sampling from the antenatal OPD and labor room of IMS and SUM Hospital, Bhubaneswar. Participants were equally allocated to an experimental group ( n = 50) and a control group ( n = 50). The experimental group received a structured midwifery-coordinated care intervention delivered through five modules at 20–22, 28–29, 32–33, and 36–37 weeks of gestation, with a final session before delivery, while the control group received routine hospital care. Data were collected using a structured sociodemographic and obstetric pro forma, the Satisfaction of Mothers with Midwifery Care Questionnaire, WHOQOL-BREF, and the Medication Adherence Report Scale. Results: After intervention, the experimental group showed significantly higher maternal satisfaction ( t (98) = 5.11, P = 0.001), quality of life ( t (98) = 8.31, P = 0.001), and medication adherence ( t (148) = 2.12, P = 0.035) compared to the control group, along with significantly lower DBP at 32 weeks ( t (148) = 4.83, P = 0.001) and higher maternal weight at 20 weeks ( t (98) = 7.44, P = 0.013). Conclusion: The midwifery-coordinated care model significantly improved maternal satisfaction, quality of life, and medication adherence among women with PIH, highlighting its potential to enhance maternal and fetal outcomes.
