Introduction This study aims to investigate the effect of Pilates exercise on static and dynamic balance abilities, as well as balance confidence, in elderly individuals aged 60 to 69 years following a 12-week Pilates exercise intervention. Methods The 43 participants were recruited from the Chongqing Hongshan Community Elderly Care Service Center, China. They were randomly assigned to the control group (22 persons) and the experimental group (21 persons). The experimental group spent 12 weeks performing specially designed Pilates exercises in addition to their normal daily routine, while the control group only performed their normal daily routine. Before and after the experiment, participants were asked to perform four tests to assess static and dynamic balance, as well as a questionnaire to determine confidence in balance. The test data were collected and analysed. Results After 12 weeks of training, the experimental group showed significantly greater standing time, with eyes open and eyes closed, on one leg than the control group. The experimental group’s walking time on the balance beam and the transition from standing to walking were also better than those of the control group. The activity balance confidence scale score of the experimental group was higher than that of the control group. Conclusions Twelve weeks of Pilates exercise could improve balance ability and balance confidence in the elderly.
