Comprehensive Wound Care Management of Multiple Stage IV Pressure Ulcers in a Patient with Paraplegia a Complex Surgical History: A Case Study

Introduction and Importance: Pressure ulcers are localized injuries caused by prolonged pressure, commonly seen in immobilized and spinal cord injury patients. Continuous monitoring and skin care (back massage) & repositioning are necessary to prevent and cure pressure ulcers. Case Presentation: A 56-year-old male patient presented to the hospital with a case of paraplegia post-spinal injury fall from the tree, with 4 pressure ulcer wounds one on the sacrum, next on both sides of the ischial tuberosity, and on both sides of the greater trochanter of the femur. His wounds were 2*2 cm, 2*2*3 cm, 5*4 cm, & 10*8*10 cm. Initially treated with antibiotics, later underwent wound debridement & flap reconstruction, was postoperatively transferred into the ward, and continued with the IV antibiotics and analgesia. The flap is well vascularized with no signs of infection or dehiscence. Clinical Discussion: Management of pressure ulcers requires a multidisciplinary approach. It begins with early identification, continuous monitoring, repositioning, comfortable devices, hygiene, diet, surgical debridement, infection control, and flap reconstruction, which are key for healing. Preventive measures such as repositioning and pressure-relieving devices & back massage help reduce recurrence. Ensuring quality care & patient satisfaction is essential. Conclusion: The back massage, skin care & flap are the best techniques for healing pressure ulcers. It has a plentiful supply of blood. Early diagnosis, surgical intervention, and preventive care are essential for effective management and improved quality of life in patients with advanced pressure ulcers.
