An Oral Health Status and Chronic Disease Care Cascade Among Adults with Diabetes or Hypertension: A Cross-Sectional Study from a Jordanian Primary Care Center

Background: Diabetes and hypertension are highly prevalent in Jordan and are linked to poor oral health. This study aimed to assess the prevalence of oral health problems and map the oral health awareness and care cascade for patients with these conditions in a Jordanian primary care setting.Methods: A cross-sectional study was conducted among 200 patients attending Alnaser Health Center in Amman, Jordan, between July 9, 2025, and November 10, 2025. Participants were classified as having diabetes/hypertension (n = 66) or neither condition (n = 134). Oral health was assessed via questionnaire and clinical examination. A care cascade was constructed for patients with diabetes/hypertension based on record review and patient report.Results: Oral health problems were more prevalent in the diabetes/hypertension group (57.6% vs. 47.8%, p = 0.192). The care cascade revealed significant gaps: only 60.6% had oral health status documented in their primary care record; 39.5% of those with confirmed problems reported ever receiving a dental referral; and only 28.9% were aware of the link between their systemic condition and oral health.Conclusion: Significant gaps exist in oral health awareness and care coordination for Jordanian adults with diabetes or hypertension. Integration of oral health screening, patient education, and clear referral pathways into chronic disease management is urgently needed.
