Stroke is one of the leading causes of acquired neurological disability in adults and is frequently associated with motor impairments and aesthetic alterations that compromise facial symmetry, function, and patients' quality of life (World Health Organization, 2021). Facial asymmetry following stroke may result from a combination of muscle paralysis, compensatory muscular hyperactivity, and soft tissue volume loss, negatively affecting facial expression, self-esteem, and social interaction. This study aimed to report a clinical case of aesthetic and functional rehabilitation of post-stroke facial asymmetry through the combined use of botulinum toxin type A and hyaluronic acid. This is a case report of a 43-year-old male patient with a history of surgically treated hemorrhagic stroke who presented with facial asymmetry resulting from temporal volume loss and compensatory muscular hyperactivity. An individualized therapeutic protocol was implemented, combining botulinum toxin type A injections to modulate muscle activity with hyaluronic acid filler to restore soft tissue volume and facial contour. Following treatment, improvements were observed in facial symmetry at rest and during facial expression, with reduced compensatory muscular hyperactivity, partial restoration of aesthetic balance, and enhanced facial harmony. The patient also reported improved self-esteem, self-confidence, and satisfaction with his appearance. In conclusion, the combined use of botulinum toxin type A and hyaluronic acid appears to be a safe, minimally invasive, and promising approach for the aesthetic and functional rehabilitation of patients with post-stroke facial asymmetry, contributing to the restoration of facial harmony, function, and quality of life.
