Worum geht es in dieser Arbeit?
INTRODUCTION: Cervical cancer is a prevalent malignancy in Brazil, primarily caused by persistent infection with high-risk Human Papillomavirus (HPV). OBJECTIVE: This review synthesized evidence regarding the diagnostic accuracy of conventional cytology compared to HPV-DNA testing, supporting the transition to molecular screening in Brazilian guidelines. MATERIALS AND METHODS: A systematic search was conducted in MEDLINE, LILACS, and SCOPUS for studies published between 2015 and 2025, analyzing 17 selected investigations. RESULTS: Data show the superiority of HPV-DNA testing, with sensitivity between 92% and 98.1% for high-grade lesions (CIN 2+ and CIN 3+), compared to 48.5%–73.3% for cytology. The molecular test showed a negative predictive value close to 100%, allowing for safe five-year intervals. Brazilian studies indicated that HPV-DNA screening detects lesions approximately ten years earlier than the cytological model. FINAL CONSIDERATIONS: Replacing cytology with HPV-DNA testing represents a significant public health advancement, providing better early intervention and reducing mortality. In conclusion, the molecular method is more effective and secure, aligning Brazil with international oncological control standards and ensuring greater health system efficiency.
