Balance between evidence, norms and discourse

Slavko Rogan

Abstract Balance training is a core component of physiotherapy. Despite extensive research, education, practice, and guidelines continue to be dominated by the assumption of a general balance ability that can be improved through non-specific training and measured using standardized assessments. The empirical evidence consistently shows that training effects are largely task‑specific and concentrate on the trained demands, while transfer to other tasks remains limited. Building on this evidence, the present paper ad …

Radiation-induced lung injury in the era of combination anticancer therapy: a narrative review

Xiuqiong Chen, Yi Duan, He Xiao, Zhengbo Wang, Xiao Yang, Chuan Chen, Mengxia Li

Radiation-induced lung injury (RILI) remains a major dose-limiting toxicity of thoracic radiotherapy. In the era of combination anticancer therapy, its recognition and management have become more difficult. Systemic treatments can cause pneumonitis or interstitial lung disease on their own, but they can also change how radiation injury evolves. By sustaining inflammation or delaying repair of the irradiated lung, these agents may convert a typical radiation-related injury into a more complex pulmonary event. For th …

Sandro Gentile, Raffaella Fiorentino, Maddalena Lettieri, Giuseppina Guarino, Giampiero Marino, Elisabetta Tommasi, Vera Frison, Ersilia Satta, Maria Chiarello, Giuseppe Caccavale, Emilia Masuccio, Felice Strollo

This response letter addresses criticisms from a recent commentary regarding a real-world survey on insulin pen needle preferences. The authors clarify that their study was purposefully designed as an observational survey to capture genuine patient perceptions under routine clinical conditions, rather than functioning as a randomized controlled trial. Consequently, methodological constraints such as blinding were fundamentally inapplicable. Furthermore, the authors emphasize the limited relevance of the Hawthorne e …
