要旨 【目的】 特定行為看護師による人工呼吸器離脱プロトコール（nurse–driven weaning protocol: NDP）と医師主導離脱（physician–led practice: PLP）の安全性を，傾向スコアマッチングを用いて検討した。 【方法】 単施設前向きコホート研究（2024年4月～2025年6月）。ICUで24時間以上人工呼吸管理を要した成人254例を対象とした。1:1最近傍マッチング（78ペア）後，主要評価項目は抜管後72時間以内の再挿管率，副次評価項目は人工呼吸日数，ICU在室日数，人工呼吸器関連肺炎（ventilator–associated pneumonia: VAP）発生率，院内死亡率とした。 【結果】 再挿管率はNDP，PLPともに6.4%（P > 0.99）と差はなかった。人工呼吸日数はNDP群で有意に短縮していた（2日vs. 3日, log–rank test: P = 0.031）。Cox回帰分析では，NDPは人工呼吸日数短縮と関連していた（ハザード比 1.57, 95%信頼区間 1.03–2.40, P = 0.037）。ICU在室日数，VAP発生率，院内死亡率に有意差はなか …
